Overview: Population health improvement strategies rely increasingly on data-driven, HIT-supported interventions. The Care Continuum Alliance, a PCPCC Executive Committee member, has developed a framework outlining the role of various health information technologies to drive various components of population health interventions. Tracey Moorhead will outline core components of population health management and the supporting health information technologies. The technologies identified extend beyond electronic medical records to encompass a wide spectrum of innovative technology devices and applications.

Biography: Tracey Moorhead is President and Chief Executive Officer of Care Continuum Alliance. Care Continuum Alliance convenes all stakeholders providing services along the continuum of care toward the goal of population health improvement. These care continuum services include strategies such as health and wellness promotion, chronic care management and care coordination. Care Continuum Alliance represents more than 200 corporate and individual members in promoting the role of population health improvement to raise the quality of care, improve health outcomes and reduce preventable health care costs for people with chronic conditions and those at risk for developing chronic conditions.
