I. Collaborative Announcements
 

A.  How to get Involved with the PCMH National Movement and the PCPCC

B. New NASHP Publication: State Strategies for Care Coordination, Case Management, and Linkages for Young Children: A Scan of State Medicaid. Title V, and Part C Agencies

NASHP conducted this scan of states in order to better identify and understand what states are doing through variously funded early child health and development agencies to promote better care coordination, case management, and linkages to services for young children. This paper describes state models that may help policy makers become more aware of the potential resources and tools available to promote healthy development for young children and can provide states with strategies to more effectively coordinate resources and achieve better outcomes for their children. C.  New Learning Collaborate to Prepare Clinicians for Better Chronic Care

HealthSciences Institute will sponsor a new learning collaborative for health care professionals, teams and organizations who serve individuals at risk of, or affected by, chronic diseases in employer, health plan, medical home and other provider settings.
The collaborative will offer free, noncommercial webinars on topics, solutions and case studies in chronic disease prevention, management and care improvement. Each webinar will include a brief presentation, ask-the-expert segment, and targeted discussion on the application of new learning to participant job roles. Discussion will continue online between sessions.
The collaborative is a component of a new HealthSciences Institute-sponsored Partners in Improvement initiative that will offer free online chronic care improvement resources and tools to health care organizations and professionals.
“Nurses and other professionals who serve at-risk individuals—in settings from health plans to medical homes—want affordable, noncommercial learning activities that prepare them for the real-world challenges of chronic care. They also understand that some of the most valuable lessons are learned through collaboration and problem-solving with peers,” cites Blake Andersen, PhD, President and CEO of HealthSciences Institute.

D. Medical Home Audioconference: The Role of the Hospital in the Medical Home Movement, January 13th, 1PM EST

Medical Home Audioconferences will present an audioconference on the The Role of the Hospital in the Medical Home Movement on Wednesday, January 13, 2010 from 1:00 pm to 2:30 pm (Eastern).

There seems to be clear evidence that a Patient-Centered Medical Home will reduce unnecessary hospitalizations and emergency room visits. Hospitals can reap financial benefits from these outcomes in cases where the patients are uninsured and would wind up as charity care, as well as in cases where the care team at the medical home helps an indigent or disabled patient qualify for government insurance. But there are other areas beyond just finance where hospitals can see a symbiotic relationship with the medical home movement - offering IT support as a way of attracting or rewarding hospital-affiliated physicians, strengthening the hospital's managed care network by partnering with high-quality physician practices, and "branding" the hospital as a caring, community-minded, patient-centric organization as well. E.  New Resources from the Healthcare Intelligence Network

HIN has several new medical home resources available that may be of interest to our members, including:

An online interview with Diane Littlewood, R.N., and Joann Sciandra, R.N., both regional managers of case management for health services at Geisinger Health Plan, on effective case management in the medical home

In this interview Littlewood and Sciandra describe case managers as the backbone of the Geisinger Health Plan (GHP) Health NavigatorSM program, which is a medical home partnership between primary care providers. Through the program GHP that has reduced 30-day hospital readmissions by 15 to 20 percent. Littlewood and Sciandra cover benchmarks for case manager caseloads and contact frequency, tools to support the case management effort, the key to smooth placement of case managers in the medical home and tips for better management of patients discharged to nursing facilities.

F. Kaiser Permanente Colorado "Medication Therapy Management in the Patient-Centered Medical Home." - January 6, 2010 webinar

Medication non-adherence is a bitter pill to swallow for the healthcare industry, accounting for more than $100 billion in costs and 40 percent of hospital admissions. From free and discounted prescription drug costs to in-office medication reconciliation, medical home programs are working hard to ensure patient medication compliance. 
 
Kaiser-Permanente Colorado will describe the responsibilities of the physician, pharmacist, patient, health IT, financial incentives and reimbursement in its medication therapy management program during "Medication Therapy Management in the Patient-Centered Medical Home," a 45-minute webinar on January 6, 2010 at 1:30 Eastern sponsored by the Healthcare Intelligence Network.

G. NCQA Seminar - How to Facilitate Patient-Centered Medical Home Recognition: A Hands-On Approach and Analysis Through NCQA's Eyes

January 25 - 26, 2010
Join us January 25th and 26th as expert faculty from NCQA guides participants in this interactive workshop. NCQA's President Margaret E. O'Kane will conduct the welcome and opening remarks for this very special program.

The faculty will examine how practices can demonstrate that they meet NCQA's PPC®-PCMH™ requirements and discuss sample submissions for PPC-PCMH Recognition. Participants will learn how to identify documentation that does (and does not) meet the requirements and we'll practice scoring each element. The group will identify strategies for enhancing and improving valid content and clarity of the application towards a Recognized Patient-Centered Medical Home and identify the aspects of the survey and evaluation process that may challenge or facilitate an efficient application and survey submission in a variety of practice environments.

This program is designed for any health care professional or consultant interested in getting the inside perspective directly from expert faculty from NCQA in evaluating a primary care practice and their ability to become Recognized as a Patient-Centered Medical Home by NCQA.

"Payment Reform — The Need to Harmonize Approaches in Medicare and the Private Sector", Peter V. Lee, J.D., Robert A. Berenson, M.D., and John Tooker, M.D., M.B.A, New England Journal of Medicine, 'In the midst of heated debate over health care reform, there is an emerging consensus that the way we pay for health care — with our widespread reliance on fee-for-service payment models — is a core problem that must be fixed. Unfortunately, too many of the policies proposed as part of reform seem to reflect the magical thinking that if we only “change Medicare” then all will be right in the world. Medicare is indeed the largest purchaser of health care in the United States, but at $414 billion out of almost $2.4 trillion, it represents only 19% of total health care spending (see graph).1 Medicare must help lead the effort to change payment, but if we’re going to create a higher-value system overall, we need to change how all public and private payers reimburse for services.' In order to make best use of our membership base and resources the Collaborative has restructured, our various task forces and projects into more formal Centers. This transition has shifted the scope of work for the Collaborative and expanded the mandates for the various subgroups. Below, please find a brief list of the functions for the Centers, for a more detailed descriptions and goals of each Center please follow the linked Center names.

  • Center for Multi-Stakeholder Demonstration: Identify community-based sites to test and evaluate the concept; share information and best practices about pilots within a collaborative community; and serve as the connector to technical, quality improvement and education resources to facilitate ongoing demonstrations.
  • Center to Promote Public Payer Implementation: Assist public payers as they implement and refine programs to embed the Patient Centered Medical Home model by offering technical assistance; sharing best practices and giving guidance on the development of successful funding models.
  • Center for Employer Engagement: Create standards and buying criteria to serve as a guide and tool for large and small employers/purchasers in order to build the market demand for adoption of the Medical Home model.
  • Center for eHealth Information Adoption and Exchange: Evaluate use and application of information technology to support and enable the development and broad adoption of information technology in private practice and among community practitioners.
A. Center for Multi-Stakeholder Demonstration
 
So far on these calls, we have received updated information from medical home projects in various stages of development across the country. Some groups have been working on practice transformation, and assisting physicians to provide services called for in the joint principles. Others have involved payers and are working to develop a system of reimbursement that uses quality measurement tools to combine a performance component to physician reimbursement.
 
On Tuesday, December 15th, CMD had a call, featuring:

II. Presentation -Dr. Renee Kanan, Regional Medical Director, California State Prison System
 
Dr. Renee Kanan received a medical degree from Washington University in Saint Louis, Missouri and master degree in public health from University of Washington in Seattle, Washington.
 
Dr. Kanan is a board certified internist who has served in physician leadership roles including medical director and chief medical officer positions within correctional settings for more than twelve years.  In addition, she has served as a correctional health care system advisor to various organizations including the RAND Corporation.
 
She currently is working in the California prison system as a regional medical director and serves on the Board of Directors of the National Commission on Correctional Health Care as the American College of Physician liaison.
 
Dr. Kanan came onto the call to discuss implementing the PCMH in the California Prison Health Care System.

B. Center to Promote Public Payer Implementation
 
With the expansion of the former State Medicaid Working Group this Center is currently in the process of reassessing our short and long-term goals. We plan to retain our current focus on state Medicaid programs, however we also plan on approaching implementation of the PCMH model in public payer programs from the perspective of the state as an employer, and federal health programs such as Medicare and the Veterans Administration.
 
On Tuesday, December 15th, the Center to Promote Public Payer Implementation had their monthly call.

D. Center for eHealth Information Adoption and Exchange

The Center for eHealth Information Adoption and Exchange will serve a number of related functions. The first will be to act as a clearinghouse for information concerning the national development of various Health Technology system platforms and electronic delivery platforms for medical records. The second task is to coordinate national education concerning the importance of HIT/EMR developments to both providers and consumers of health care. The final task of the Center will be to elucidate the integral role of HIT/EMR development within the specific context of the Patient Centered Medical Home model and expand upon the provision with the Joint Principles of the Patient-Centered Medical Home as agreed to by the ACP, AAFP, AOA, and AAP.
 
The Center for eHealth Information Adoption and Exchange met on Thursday, December 10th, at 1:00PM EST. 
 
The call featured, among other items: 
  1. Welcome and introductions - Dr. David Nace
  2. Presentation - Jonathan Klein, MD, American Academy of Pediatrics.  Dr. Klein has been recently appointed a new full-time leader at AAP and has been tasked with development of their HIT strategy in support of their mission and membership.  Hear Dr. Klein describe the history of what has been done at AAP to date as well as his thoughts on how AAP can best support their members, as well as children and families, with regard to using HIT in a PCMH model to facilitate practice transformation and improvement in health outcomes.
