CMD Mission

The mission of the Center for Multi-Stakeholder Demonstrations (CMD) is to develop and advance the patient-centered medical home (PCMH) through our role as convener and supporter of demonstration projects and pilot programs designed to field the PCMH in communities, regions and states. The CMD is to serve as a dissemination center for information on PCMH pilot efforts around the country.
