The I3 PCMH Collaborative is a practice transformation collaborative of 25 academic primary care teaching programs in NC, SC, and Virginia. Modeled after the IHI Breakthrough Series Collaborative methodology, teams from each of these sites participate in monthly Webinars, electronic Blackboard web site data posting and information sharing, and in four face-to-face meetings (2-days each) in order to share and learn from each other and from invited keynote speakers. Topics generally follow the NCQA Standards for PCMH Recognition, but also include additional topics such as behavioral integration, health literacy, transitions of care, etc.

The goals of this collaborative are:

1.) To provide a forum to propel practice transformation to PCMH models in teaching practices which train primary care physicians who will populate practices in the community.

2.) To assist each of the participating practices in achieving NCQA recognition for their teaching practices

3.) To develop and study methods of curricular innovation and integration for the purpose of enhancing medical education around training for the PCMH

4.) To study the outcomes of practice transformation in academic teaching programs

5.) To work with payers and other stakeholder to improve the ability to sustain the practice transformation that is underway
