Hoangmai Pham, MD, MPH, Senior Health Researcher, Co-Director for Quantitative Research, Center for Studying Health System Change – Dr. Pham will speak about a new environmental scan on service agreements and compacts - what has worked and what has not worked to date and what are some policy options; additionally she will discuss current health care reform thinking covering PCMH and Accountable Care Organization.
R. Scott Hammond, MD, FAAFP, Associate Clinical Professor in the Department of Family Medicine at the University of Colorado Health Sciences Center  – Dr. Hammond will speak at a very practical level about his success working with specialists and hospitals in Colorado to facilitate greater care coordination.
Deborah Emery, Board Member, Patient, DFD Russell Medical Center, an FQHC in our Maine PCMH Pilot - Ms. Emery, the principle of Henry L. Cottrell School, will speak about her experience as a patient in her community hospital that led to her taking an active role to improve care coordination and patient-centered care.

Allan H. Goroll, MD, Chair, PCPCC Payment Reform Taskforce; Professor of Medicine Harvard Medical School, Massachusetts General Hospital – Dr. Goroll will speak about the PCPCC Payment Reform Taskforce and the upcoming Payment Reimbursement report.
William Rollow, MD, MPH, Solutions Leader, Healthcare Value & Transformation, Strategy and Change Group, IBM – Dr. Rollow will speak about obtaining wide spread support for a PCMH value based campaign to seek commitment from payers to pay the costs of PCMH activities that create value.
Lynne Cuppernull, Director, Clinical Learning & Innovation, Alliance of Community Health Plans – Ms. Cuppernull will speak about what community health plans are doing related to practice transformation and payment reform, and present the Alliance’s findings.
Thomas H. Peterson, MD, Priority Health Dr. Peterson will speak about their pilot which seeks to demonstrate the value of specific attributes of the PCMH: access, care coordination, and patient engagement.

Regina M. Benjamin, MD, MBA, 18th Surgeon General of the United States Public Health Service- Dr. Benjamin will talk about how the medical home is a place where patients feel comfortable and safe; a place to talk and learn and build a long-term, trust-based relationship in the primary care setting.

Donna D'Alessandro, MD, Co-Director, Educational Informatics Laboratory, University of Iowa Health Care - Dr. D'Alessandro, nationally recognized as an expert, will speak about her experience as pediatrician at the University of Iowa Children's Hospital and about the creation and evaluation of online educational resources and digital libraries. Additionally, she will speak of the Decision Support Taskforce research paper.
Jerome Osheroff, MD, Chief Clinical Informatics Officer, Healthcare Business of Thomson Reuters, Adjunct Faculty and Clinical Staff, University of Pennsylvania Health System - Dr. Osheroff will discuss the use of decision support and quality improvement analytics to support ambulatory practice.
Steven Waldren, MD, MS, Director of Center for Health Information Technology, American Academy of Family Physicians - Dr. Waldren will talk about his MD practice and the meaningful use of HIT tools.

Mark Fendrick, MD, Professor, Internal Medicine and Health Management & Policy, University of Michigan; Center for Value Based Insurance Design; Co-Editor in Chief, American Journal of Managed Care – Dr. Fendrick will deliver the new PCPCC’s Value-Based Insurance Design and the Patient-Centered Medical Home Guide and he will highlight the employer access section.

Cynthia Napier Rosenberg, M.D., Senior Medical Director, Network Management and Provider Relations, UPMC Health Plan - Dr. Rosenberg will discuss the results of UPMC's PCMH Pilot.
