PCPCC/NASHP State Roles in Multi-Payer Medical Home Pilots - November 12th

This is the third in a series of web seminars on the patient centered medical home concept and the value it brings to primary care. This program focused on existing multi-payer medical home pilot programs, covering defining characteristics of each, reimbursement methodologies, how the pilots are supporting practice transformation, and the state role in the effort.  This event is due to a grant from the Commonwealth Fund.
