In early 2010, 5 Clinics began a Medical Home Pilot Project. Project goal is for each Clinic to obtain NCQA Patient Centered Medical Home Recognition. All Clinics developed an Action Plan incorporating specific NCQA Standards/Elements. The only project requirement was to include Standards focusing on Clinical Conditions and Referral Management since these are two key areas of focus in the Portland market. In 2011, the Project will expand to add approximately 25 more Clinics.
