Join the Center for Multi-Stakeholder Demonstrations for a free webinar on June 1st at 2:00 PM EST. The webinar will last 1 hour and 30 minutes. Following the speaker's presentation, there will be time allotted for Q&A.

Description:

Stakeholders from the Colorado pilot will cover the structure of their pilot, milestones, measures, data, and the technical assistance provided to participating practices. A patient will speak about her involvement in the pilot, a practice and hospital will share their work on strengthening communication between the hospital and practice/provider and a practice care coordinator will talk about the work being done to improve coordination of care within their medical neighborhood.
