The Primary Care Information Project
The Primary Care Information Project seeks to improve population health through health information technology and data exchange. The program supports the adoption and use of Electronic Health Records (EHRs) among primary care providers in New York City's underserved communities. There are more than 2,500 providers currently using the system, making it the largest community EHR extension project in the country.
The webinar will cover:
- The unrealized promise of EHRs as quality improvement tools
- EHR functionality for enhancing practices' capacity to provide patient-centered, continuous, coordinated care and to improve population health
- A brief history of the New York City Primary Care Information Project (PCIP) and an overview of the suite of services available to PCIP members
- Insights from the QI Team: Learn how the Quality Improvement Team works with primary care physicians to help them become Patient-Centered Medical Homes and prepare for Meaningful Use, including lessons learned along the way and the challenges they face
- PCIP’s next steps and plans for the future

Joslyn Levy is the president of Joslyn Levy & Associates. The firm provides technical assistance, expertise and project management to foundations, public health agencies, clinical providers, and other key stakeholders dedicated to improving individual and community health. Before establishing her consulting practice, she served as the Director of Clinical Systems Improvement at NYC’s Department of Health and Mental Hygiene, leading large scale quality improvement initiatives. Ms. Levy played a key role in the development of the Primary Care Information Project, advising on the inclusion of critical quality improvement tools, such as registries and self-management support tools, and designing strategies for EHR-enhanced quality improvement.

Ms. Nikas, a quality improvement specialist with the NYC health department’s Primary Care Information Project, helps primary care physicians use EHRs to improve the health outcomes of patients and prepares the practices for potential incentive programs. She is a Registered Nurse and holds a BA in Health Sciences from Brooklyn College.
