About:

The National Center for Medical Home Implementation is a cooperative agreement between the Maternal and Child Health Bureau (MCHB) and the American Academy of Pediatrics (AAP). The mission of the National Center for Medical Home Implementation is to work in cooperation with federal agencies, particularly the MCHB, and other partners and stakeholders to ensure that all children and youth, including children with special needs, have access to a medical home. • Enhancing policies and operational standards that guide implementation of the medical home through partnerships at the national level. • Providing resources and tools that increase implementation of the medical home at the practice level. -Enhancing the collaboration for medical home system change at the state and community levels. • Leveraging the AAP structure—chapters, committees, councils, and sections—to further maximize medical home implementation at all levels from local to national.
